Opportunity Registration Form

Pursuant to Section 19.0 of the International Reseller and Distributor Agreement

INSTRUCTIONS

  1. Click “Download Form” below to download the Opportunity Registration Form.

  2. Complete all fields in Sections 1–5. Incomplete forms will not be processed.

  3. Submit by email to [email protected] with subject line: “Opportunity Registration — [End User Name] — [Distributor Name]”.

  4. CIRCADENCE will respond within five (5) business days with an Approval, Rejection, or request for additional information (§19.1).

  5. Approval is confirmed only by written notice from an authorized CIRCADENCE representative. Silence does not constitute approval.

  6. Upon approval, a six (6)-month Exclusivity Period commences from the date of written approval. Extension requests must be submitted no later than fifteen (15) days before expiry (§19.3).
